This example comes in the heels of a similar white shirt (used between 1996 and early 1997) that had the same stripe on front but also had different collar and cuffs, and featured the Mutual Alajuela sponsor on black text on white background.

This shirt also had a further variant in which it had the same front but also had the boxed Mutual Alajuela sponsor in the upper back, and part of the red/black stripe was featured in the lower back as well.

Shirt #5 was used by midfielder Luis Diego Arnáez.
