Swansea City on 13/08/94, final score 1-1. Kurt Nogan scored for the Albion. The number 4 shirt was worn that day by Ian Chapman.
Wrexham on 27/08/94, final score 2-1 to Wrexham. Junior McDougald scored for the Albion. The number 4 shirt was worn that day by Jimmy Case.

Another away shirt influenced by Liam Brady, never saw proper competition because the league deemed it too similar to the home shirt(!?)
I believe it was only worn once, for a pre-seaon friendly against Spurs. Given that the final score was 7-1 to Spurs, maybe it was best that it got binned.
